CPT® - 99201 denied

DX Billed - 739.1, 728.85, 739.2 & 739.3
Date of service: 09/19/2012.

Information from UHC:

I called UHC a rep said the Claim for the code 99201 is denied stating that the services are not payable on the usage of incorrect modifiers. Did i bill the claim with incorrect Informations.
