Does anyone know the 2012 CPT® code for Bicillin LA 3,600,000 units? Does it need a modifier as well as the code?

J0561 (100,000 units). You must include units per 100,000 to dose administered.

If I use J0561 for a Bicillin LA injection of 3,600,000 units would it be 100,000 times 36? This is what I used but ins only paid for 1 x 100,000 units. What else do I need to include?
