Lipoma removal

Our physician is excising a large lipoma on the back, 4cm. However, he is making a smaller incision, 2cm and going in cutting the lipoma away and then removing that thru the smaller incision. We know the close will be coded 12031. The question is how do we code the excision. He is still removing a 4cm lipoma, so would the excision code be 11404 or do you code it based on your incision length, 11402?
