independent living

If physicians go to a facility that have skilled nursing, assisted living, and independent living patients to provide medical services in that facilities medical office and they see independent living patients what codes and place of service should be used? The independent living patients are seen at that facilities medical office not in their private residence. Also, they are not provided with any care/supervision of any kind from the facility. Any assistance with which codes and place of service appropriate would be greatly appreciated.
