Therapy re-eval 97002 and 97004

I am unable to get any of the reevaluations paid even by themself or with other services. I have added the -25 modifer and they have been denied. This goes for all insurances including MC and Anthem. Can someone help so I can get these paid. Thanks

I would first call the carrier since some do not pay for re-evals (97002/97004). If they do pay, then you will know what needs to be done because then you will ask why they didn't pay it. Someone else may be more familiar with these particular carriers to give you a more exact answer, but you can get the same exact answer by calling the carrier and asking them why they denied it. What does the EOB say?

What does the EOB say, why was it denied? Because anthem usually does pay for this service.
