Modifiers for Needed Units

Hi
For the life of me i cannot figure out what modifier to use in this instance.

I billed for an oral appliance but the insurance co is saying that all units are filled. But if i were to use the correct modifier they would pay it.

all units maxed out, but if i were to use the right modifier they would pay it.

Can you help, please?!
