Coding for screening CXR

In addition to pre-procedural phrasing, is a clinical ICD code needed for screening CXR such as for
(i) pre-admit CXR for appendicitis in an otherwise healthy 10 years old girl
(ii) pre-op CXR for left humeral fracture in an otherwise healthy 54 years old man
(iii) pre-transfer CXR to SNF (skilled nursing facility) in a 90-years old senior with no cardiorespiratory symptoms

I am aware that "hospital/surgical/SNF policy" are not in and of themselves reimbursable conditions.

What is your reference document for these items?
