Billing for telemedicine or electronic consults

Hi,
I would like to understand how you bill for patients that the doctor does not see in person. For instance, how would you bill if the doctor or patient were communicating via:

* Telephone
* Video conference
* Email/secure messaging system.

I have just posted a question on specifics for telephone services as well, to get a better understanding... but they do have the CPT code for telephone services as 99441-99443 and On-line Medical Eval 99444. I also did a google search for cpt coding guidelines for telephone services and the AAFP has some good articles in there
