Fecal Blood Occult tests for Medicare
We keep receiving denials for the fecal blood occult tests. The denials are CO-16 stating that the CLIA number is missing or invalid. I've been told to code G0328 instead of 82270 but both are being denied the same. I've also been told to use the Mod -QW and its still denied for the same reason... Can anyone please tell me what we're doing wrong so we can close out these claims!?

Fecal occult blood testing (FOBT), as its name implies, aims to detect subtle blood loss in the gastrointestinal tract, anywhere from the mouth to the colon. Positive tests ("positive stool") may result from either upper gastrointestinal bleeding or lower gastrointestinal bleeding and warrant further investigation
