Billing for pump

The nurses feel that I should bill a 96409 along with the 96521 because they pushed the residual from the pump. I say no!

Hi,
When you bill to the insurance companies other than Medicare do you add modifiers to the refill code and supply code? We are receiving denials for them being included in the procedure.
