Skilled Nursing home facility discharge codes for Medicaid Patients?

Hi I am a billing specialist in our organization for physician services. Recently we are getting denials for Medicaid patient who got discharged in Skilled Nursing Facility. We are using 99315 & 99316. All the other insurance companies are paying for this CPT® code but Medicaid is denying stating it is "not a covered benefit". They are asking us to contact the coder and resubmit the claims. What CPT® code can we use for the discharge service by a physician @ Skilled Nursing Facility for Medicaid Patient.
