Colonoscopy

The doctor order reads "history of polyps". On the procedure report he puts "Screening". The outcome - no polpys are found.
What would be the correct admit and primary diagnosis?

I am assuming the hx of polpys were adenomatous polyps which you could do a 2 year screening V12.72. Even if polyps were hyperplastic most commercial payers will pay V12.72.
