Are there any particular rules; do's/don'ts when using this modifier 57 (decision for surgery). Thanks.

Modifier -57 should be appended to any E/M visit that resulted in the decision to operate, if this visit happened the day before, or day of the surgery. Otherwise the visit will be denied as global to the surgery. This is for major procedures, so decisions for minor procedures (10 day or no glabal period), should have the -25 modifier instead.

I guess I should clarify that not every minor procedure will get you an extra e/m code with a -25...this is usually included.
