billing for chiropractic with acknowledment and discolsure form

Hi ,
I am in a pickle. I had a chiropractor call and want my billing services just for his personal injury claims..i would like to know how do i go about doing the claims with originial acknowledgment and disclosure form will i have to send all the claims in by paper? Or do i file them electronically and then mail the form to the insurance company? HOw long do i have to file these claims? Thanks so much!!!
