When can an interval history be used?
1. within 30 days of discharge
2. within 30 days of discharge for same readmission

interval history
As described in subsequent hospital visit codes, these codes describe the interval history meaning what has happened to the patient in the past 24 or 48 hours, or last seen in hospital.
Interval history
This is a specific term used in the CPT book to describe the history required of subsequent hospital and nursing facility visits.
The definition of it is found in the Documentation Guidelines. It is not time related as it pertains to what is required in an E/M note.
