Modifer 22 documentation

One of the physicians in this practice has been told that in order to use a modier 22 there has to be two seperate documentations, can you tell me if a seperate statement on the operative note will suffice or does there have to be a stand alone document in order to use modifer 22? Thank-you

Not true. At 22 modifier should be supported in the documentation. The provider should dictate in the note what it was that made the procedure more extensive than it would normally be. Only 1 dictation is needed.

The role of the -22 modifier is to reflect additional work that is not typically part of the procedure, but does not qualify for its own procedure code. Prior to using the -22 modifier, please evaluate the description of the increased procedural service to determine whether there are other procedure codes to account for the increased work.

I send the documentation and highlight the reason for 22 modifier but I also attach a note stating the I am requesting additional reimbursement and the reason for it. This is signed by the physician.
