billing for temp pacemaker, stent, cath

I am billing a stent, a left heart catheterization and a temporary pacemaker on the same date of service.
I am adding a modifier of LC on the stent,
a modifier of 26,59 on the heart cath.
Would I add a modifier of 59 on the temporary pacemaker as well?
Medicare is rejecting the claim (M80), not covered same session.
