26445 & 26525

I billed CPT code 26525 for a long finger capsulectomy and code 26445 on the same finger for tenolysis and the insurance company is denying as incidental to each other but according to AAOS guidelines 26445 is not included with 26525. I appealed with the guidelines but they are still stating incident. How can I appeal this again?
