G0477 / g0478

I've always used the modifier QW with G0434 because we are CLIA waved. Medicare has denied the new code G0478 - QW saying the modifer is wrong. I can't find anything telling me which modifier to use instead. Also, does G0477 or G0478 replace G0434? I've been dealing with this for three weeks now and can't figure it out. Can anyone PLEASE HELP???
