Newbie looking for appropriate CPT for OP note

Being new to surgical coding, I am having some difficulty finding the appropriate CPT code for this surgery and need help.

My surgeon performed a partial gastrectomy on a gastric remnant and a tube duodenostomy. I am not sure which code I should be using. The pt is S/P Roux-En-Y bypass.

I found 43631 (partial gastrectomy with duodenostomy). I cannot seem to find a more appropriate CPT code. Since the patient is S/P bariatric surgery, I do not feel that 43633 (partial gastrectomy with Roux-en-Y reconstruction is appropriate).

CPT code 49441 is insertion of duodenostomy tube but performed under fluoro. No where on the OP report, is this stated that the tube placement was performed under fluoro.

Would I maybe use the 43631 code with a 52 modifier and the code 49441 for the duodenostomy tube or just use 43631????

Please help!

bariatric surgery

As with all surgeries medical necessity diagnosis is required. If in a global period and patient developed a complication, use modifier -78. Otherwise by your description, use code 43631, based on op-note dictation.
