Does the chief complaint have to be documented by the provider or can it be done by the ancillary staff?

Technically, the Provider. The nurses can enter preliminary information, but the provider must add or modify it. It is especially important that the provider adds or modifies in his handwriting, or in electronic records because the audit trail must show he/she did address the Chief Complaint.
