Nurses visit 99211 vs. Admin code 96372

#1 The patient comes in regularly for a pain injection for migraines. Does not see the doctor, only the nurse. Can we bill a 99211, 96372 and medication codes?

#2 A patient comes in regularly for injection of testosterone and brings his own meds. Only sees the nurse. Can we bill both codes or only the admin code?

#3 Patient comes in for allergy injections. Sees only the nurse. Nurse observes for the required amount of time to be certain there is no reaction. Both codes or only one?

Any help will be appreciated. These are not test questions. This is an ongoing issue in our office between the provider and the coder.
