cardiology s/p cabg coding

I'm a coder for a cardiology practice and we have many patients that have ASHD s/p CABG. Is it ever appropriate to code as I25.10 and Z95.1 or would I just use I25.810 because it encompasses both? This is assuming that patient did not have angina. ICD9 had the 414.00 and V45.81, not one code for both. When is it appropriate to use the Z95.1 without the ASHD code? Thanks.
