shared office visit

When both the physician and his PA examine a patient, in order for the billing to be under the physician's name, does the physician have to perform and dictate the physical examination or just the impression and plan?

Shared Visit

Shared visits can be billed in the office setting, but Medicare requires that the "incident to" billing rules be met before a shared visit is billed in the office setting. other than incident-to, do not consider shared visits in office site of service. In most cases, shared visit billing is more appropriately utilized in the hospital setting. In the office setting (not incident-to), if the physician is billing, must perform the HPI, Physical and MDM. Ancillary staff can do ROS and PFSH but physician must document negatives and positives of presenting problem(s).
