What code(s) would a physician bill in range 95145-95149 if they are preparing for a patient one vial with 2 venums and one vial with 3 venums different dosages in each? Would they code 95146 and 95147 with their individual doses in the units field or would they use 95149 with the total doses listed in the units column?
