CPT® code 94375 denied
I am getting denials of CPT® code 94375 as having a missing or incorrect modifier. What is the correct modifier to use? We are not using with 94060.

What is your place of service? If a facility setting, use -26.

bill with a TC modifier and the read doctor with bill with the 26 modifier.
