CPT® 76882

Hi! We are billing CPT® 76882 with valid dx a/p lcd. The claim is getting denied by Medicare as CPT® needs modifier or the modifier appended is incorrect. The claim initally had modifier 26 then it was removed & rebilled. Both the times we recieved same denial. Found no other CPT® billed for dos or global codes. Place of service 21/22/23.
please suggest a solution !!!!!!!!!!!!!
