E/M Coding for Gynecology

My physicians are feeling some confusion with the new ICD10 code and annual gynecologic examnation. We have Z01.419 which is an annual gynecologic exam with no abnormal findings and Z01.411 with abnormal findings. The question is this: Is a patient who is status post hysterectomy considered a normal annual (Z01.419). And secondly, what is considered an abnormal finding for the exam? Thanks for all your help!
