E&m

My providers are obtaining information that I am not in agreement with. They want me to bill 2 E&M codes on the same day/visit. ie; Pt has a follow up for COPD (99213-25) then stated also has had chronic headaches (99213). I have never heard of this. Is this correct? Can someone please help me?

One E/M. Combine the documentation for both problems and select the appropriate level of service.

THANK YOU. Where can I find documentation on that? I need to show them. I have looked on several insurances and CMS and am not able to find it documented.
