Is primary diag. code V58.69 inappropriate?

Hi I have a scenario of primary diag. code is V58.69 and claim was denied because it was inappropriately used. The CPT® codes are 99212, 85610, and 36416.

You need to specify the reason/condition that requires the patient to me monitored for long term medications. V58.69 should be the last diagnosis code you use.
