New tce codes

My DR has been billing for the transition of care with some of our patients that have recently been in the hospital. So far they have all denied for "Time frame between this service and related service have not been met." Patient was discharged from hospital and saw in 7-14 days in our office for followup. Any ideas as to what needs to be done to get these paid? Thanks.

Transitional care management commences upon the date of discharge and continues for the next 29 days. Cannot bill before 29 days after discharge.
