Est Office visit with a surgical procedure

When an established patient comes into the office and has a surgical procedure done, what modifier is being used with the 99211-99215 codes? I have been using the 25 modifier but, due to the new changes for 2012 that code is being denied. Thanks for any and all help.

What is the reason for the denial? What procedure are you doing? I am not aware of any over arching change that would affect this.
