denial

CPT® codes 15120 (split-thickness autograft face, scalp, eyelids, ears, etc) and 11442 (excision, other benign lesion including margins). Medicare paid for the 15120 but denied 11442 as not medically necessary. How can I get Medicare to pay for 11442? Thank you for your help.
