Super bill for billing Hospitalist charges

I am currently using an overcopied and over edited copy of a super bill for hospitalist billing.

Rather than re-create the wheel and with the 2010 cpt code changes approaching us, how can I get access to a existing super bill specific for this specialty?? I would rather not start with an excel spreadsheet and work from ground up.......

I have been searching the internet for something of the sort and am not having luck.

Many groups use books that they purchase: small, pocket size cards that are one/patient. Some clients have moved to an electronic charge posting system.
