G0168
Is this used for Physician billing only? Is it used for all payors?
Example: ER chart, Medicare, patient comes in with laceration to eyebrow. Doctor fixed it with Adhesive. Do you code the G0168, or 120xx, or both?

ER visit code with -25 modifier, G0168. Medicare use but otherwise payer specific.
