Dr wrote, "Amputation, 2nd toe, RT an LT.
This is being billed to Medicare.
Medicare states to bill as one procedure with two modifiers.
Is is OK to bill as 28820 T6 T1?

If that is what they are telling you. Also, put units "2" to describe 2 services. I would have used 2 line items with appropriate modifier; the second line item may have required the 59 modifier, ex: 28820-T1; 28820-59-T6.
