Is it okay for the physician to only have had 2 out of the 3 Past Medical, Family, Social History (PFMSH) for preventive CPT® 99396? The record does not state that patient had Family history and no comment in the record provided as to reason.

The preventive codes have no requirement for PFS history. These codes cover what is necessary on an asymptomatic patient based on age and new or established physical examination.
