CPT® 93015

I have a question most insurance denied the CPT® 93015 when we billed with 93306 and 93350. Even when we submitted with 59 modifier for 93015. It got denied as inclusive to primary can any body help me to sort out this

continuous electrocardiographic monitoring, and/or pharmacological stress; with physician supervision, with interpretation and report
93306- Echocardiography, transthoracic realtime with image documentation (2-D),
includes M-mode recording, when performed, complete, with spectral
Doppler echocardiography, and with color flow Doppler echocardiography
93350- Echocardiography, transthoracic, realtime with image documentation (2D),
includes M-mode recording, when performed, during rest and cardiovascular stress test using treadmill, bicycle exercise and/or pharmacologically induced stress, with interpretation and report;
Do not report 93350 when the stress echo and stress test are provided by the same physician in the physician office. See code 93351.
93351- Echocardiography, transthoracic, realtime with image documentation (2D),
includes M-mode recording, when performed, during rest and cardiovascular stress test pharmacologically induced stress, with interpretation and report: including
performance of continuous electrocardiographic monitoring, with physician supervision.
Report when a stress ECG and stress echo are performed by the same physician in the physician office. Replaces reporting CPT® 93015 and 93350.
