Audting HPI

Recently a discussion occurred among our auditors regarding counting HPI elements for mulitple "locations'. For example, pt is seen for URI and hip pain...is it appropriate to count 2 locations (Resp and MS) and if so, is it then appropriate to count other HPI elements more than once if they are distictly documented as relating to each system? We understand the concept of 'double dipping' but in the context of addressing completely separate systems, is it allowed?

I'd say no on that. Basically, of the 8 elements of HPI, I wouldn't support an attempt to have, say, quality twice, just related to different locations. I've never seen that allowed.
