Office setup in surgical centre

My provider is a pain medicine specialist. he has office in chandler, he also perform procedure on a surgical center. He is getting good referrals at that place now he is willing to set that location as his second office. It this possible. My questions are:
1. Can a provider set more than one office location 2. Can he bill with same location as Office and out-patient surgery POS. 3. Do we need to inform to insurances about this before starting seeing patients. 4. What are Medicare and Medicaid regulations about this.

Any other information that would help us.

PS: please note that surgical center is agree to issue a letter for him allowing to sent an office there.

1- of course
2-not clear your question. You bill place of service as office if he rents the space, employs the staff and owns the equipment. If not, it is outpatient surgery center
3- You should.
4- You must inform them for sure.

Be sure to get place of service right- this is a big audit target with fraud investigators.

Doctor is willing to set office setup in surgical centre and wants to bill claims with POS 11, but he does perform surgeries in same surgery centre, so he wants to bill office visists with POS 11 and surgery codes as POS 24, 21 or 22, Is this possible?

Yes, it is correct. The office visits are in the suite he rents and the employees work for him so POS 11 is correct. He does surgeries in a surgicenter that is not owned by him and the surgicenter will bill for the facility fee so you cannot use POS 11.
