Coding New Patients EM as Established Patient due to no exam done

Hello,

I am being advised to code a New patient E&M as an Est. Patient due to an exam not being done in the documentation. Can you please advise? CPT® Guidelines do not state this procedure nor am I able to locate any information via CMS.
