EMG's And NCS

My physician has billed a
95900 (DX of 353.2 719.42 782.0 for all lines of service.
95904
95886
95832 (this denied as bundled)
Per Encoder Pro, There are no bundleing issues.
Could it be the DX order on the 95832?
Any thoughts?

You can find the NCS/EMG determination on Medicare website LCD # 2918. It explains coverage. I bill for EMG's and have never billed a 95832. It looks like a code out of the realm of the actual test. Could it need a modifier? I'm looking forward to seeing the answer to this. Anyone have any answers?
