Medicare's Payment Policy Indicators

I need to know where I can find Medicare's payment policy indicators and what they mean....status M for example. I had this but lost it and just spent 30 min on CMS site trying to find the definitions......Help appreciated.

M status indicator

M = Measurement codes, used for reporting purposes only.
There are no RVUs and no payment amounts for these codes.
Medicare uses them to aid with performance measurement. No
separate payment is made. These codes should be billed with a
zero (($0.00) charge and are denied) on the MPFSDB.

I found it on page 1322 of the Final Rule. I know it's in one of the manuals, but I can't remember which one.
