is this for vision or medical? I know for medical it's as many times as need be; the only carrier I've ever encountered with any sort of restriction is Medicaid - in my state, the number of times is 3 per year; other than that, I haven't had any issues; you may want to check with your carriers to see if they have any limitations on frequency...

This is a Ophthalmology Comprehensive exam, it was my understanding, we could charge only after the patient was last seen at least 4 moths ago, I my be mistaken?

I have never heard of the 4-month limitation - it's not in the descriptor for this service, nor have I heard the AAO/AAOE specify any time restrictions; some carriers only like to see the 92XXX codes from optometrists & regular E&Ms from ophthalmologists, don't ask me why, they just do... I code for 5 ophthalmologists each in a different sub-specialty & I've never seen any time limit restraints for the 92XXX series except for our state Medicaid...
