Prolonged services for in patient 99356 and 99357

Is anyone familar with these codes?

For example if you have a Medicare patient, consult 99223 and you spend 100 minutes with them do you code :

99223 (first 70 minutes) Modifier ?
99356



Of say for subsequent hospital visit you spent 55 minutes with them :

99232 (first 25 minutes) Modifier?
99356 ( Spend 30 more minutes)


These are not critical care patients...
