Work comp bundling 64718 & 64721

I need someone's help with appealing these two codes. Work comp insurance keeps denying 1st dorsal compartment release as incidental to carpal tunnel procedure. I have sent two appeals with operative report, AAOS guideline info and diagrams of both procedures clearing stating seperate incisions and correct modifier 59. I have even asked for a peer to peer review but they state we can't do that. My administrator does not want me to take off the charge. Help!
