If a patient has a co-pay does it usually apply to a 99211 nurse visit?

I never collect a co-pay for 99211. We usually reserve that visit for nurse visits or blood draws. It is usually a quick visit.

Many companies require a copay for all E/M services, including 99211. But, you'll have to check with each one.
