florouscopy 77003

I am gettting denials from Anthem Blue Cross for 77003 being bundled with 64640.
These two codes are not being bundled by Palmato,Workers Comp, or Pacificare.

The cpt book does not say these codes are bundled, and the cci edits do not say they are bundled.

Is anyone else having a problem with this? Has anyone found a solution?
