suture removal by physician/nurse that did not place the sutures

If an E & M code is used for the visit what diagnosis do you use when removing sutures that were placed by another physician? Also, can they be removed by anyone other a physician?

the diagnosis code would be V58.32; if you're billing anything higher than a 99211, there needs to be general physician supervision
