Inpatient vs Observation

Medicare allows 48 hours in observation or until all ordered interventions have been completed. Example: IV ordered before 48 hours, infusion not complete until after hour 48. If ipatient is ordered, order must be amended to reflect inpatient admission. Usually a case management responsibility to make certain time limits are not exceeded.
Many managed care payers limit observation to 23 hours. In that case, depends on your contract with the payer, and requirements if converted to inpatient, which usually requires a new authorization.
